WHO MAY RECEIVE SERVICES?

The Free Medical Clinic of Northern Shenandoah Valley serves residents of Winchester City, Frederick County, and Clarke County who need medical care but do not have health insurance and cannot afford to pay for services. The FMC provides non-emergency primary medical, nursing, dental, and pharmacy services, as well as health education.

 

ELIGIBILITY PROCESS

First-time eligibility screenings are every Monday and Wednesday from 1:30 p.m. to 3:00 p.m. No appointment is necessary. Thursday evening’s screenings are available between 5:30 p.m. and 6:30 p.m. Once you are approved you will be issued a patient eligibility card. Please keep this card with you and be prepared to show it any time you seek services from the Free Medical Clinic.

 

ELIGIBILITY GUIDELINES

Proof of total household income will be required, along with other supporting documents.
Patients’ household cannot earn more than 150% of the Federal Poverty Income Guidelines.
Patients must not have any health insurance, including Medicaid and Medicare, or insurance that covers hospitalization, nor be eligible for services through the Department of Veterans Affairs.

 

ELIGIBILITY REQUIREMENTS

You must bring the following in order to qualify for services:
1. Proof of residency within Winchester City, Frederick County or Clarke County
2.Current 1040 Federal Income Tax form (if one was filed) including W-2(s) and 1099
3. Proof of total household income for three months

Examples of proof of household income are:

Pay stubs from current employer for 3 months
Food stamp award letter
Letter of support from whoever is providing shelter and food
Proof of Social Security (award letter, copy of a check OR copy of a bank statement showing deposit)
Proof of Disability income (award letter, copy of a check OR copy of a bank statement showing deposit)
Proof of Supplement Security Income – SSI (award letter, copy of a check OR copy of a bank statement showing deposit)
Proof of pension (award letter, copy of check OR copy of a bank statement showing deposit)
Proof of Unemployment (award letter, copy of check OR copy of bank statement showing deposit)
Proof of child support (copy of court order, canceled check, or receipt)
Schedule C tax forms if you are self employed
Current bank statements
